Affirmed Home Care, New York’s premier concierge home care agency, is looking for experienced Licensed Practical Nurses (LPNs) to provide exceptional one-on-one care to our clients throughout Suffolk County.

The Opportunity
 
We are seeking compassionate and dependable LPNs for highly specialized cases, with flexible 6–12 hour shifts available throughout Suffolk County.
Cases may be available in areas including:
  • Huntington
  • Smithtown
  • Kings Park
  • Surrounding Suffolk County communities
Compensation
  • $40.00 per hour for hourly shifts
  • Flexible per diem scheduling
  • Overtime opportunities may be available

Requirements
  • Minimum of 1 year of current LPN experience
  • Home care experience is a plus
  • Valid New York State LPN License
  • Valid BLS Certification
  • Valid driver’s license preferred
  • Eligible to work in the United States
  • Physical exam completed within the past year
  • PPD or QuantiFERON completed within the past year, or chest X-ray within the past 5 years
  • MMR immunizations within the past 10 years
