Key Responsibilities:
Conduct comprehensive client assessments to create personalized care plans.
Administer medications, treatments, and nursing procedures as required.
Monitor clients' health status (vital signs, progress) and adjust care plans as needed.
Educate clients & families on disease management, medication use, and home safety.
Perform advanced nursing procedures including wound care, IV therapy, and catheter care.
Document all care provided in compliance with regulatory guidelines.
Ensure compliance with state and federal healthcare regulations.
Provide emotional support and guidance to clients and their families.
Stay up-to-date with nursing best practices and participate in professional development.
What We’re Looking For:
Active RN License in the state of New York.
1-2 years of nursing experience (home health, geriatrics, or similar preferred).
Strong clinical skills and ability to perform nursing procedures independently.
Excellent communication skills and the ability to build rapport with clients and families.
Self-motivated and organized, with the ability to work independently in a home care setting.
Reliable transportation and a valid driver’s license.
CPR and BLS certification (required).
Experience with electronic health records (EHR) systems (preferred).
