Key Responsibilities

• Conduct initial and ongoing assessments to create personalized care plans

• Administer medications, treatments, and specialized nursing procedures

• Monitor clients’ health and respond to changes promptly

• Perform advanced nursing care, including wound care, IV therapy, and catheter management

• Provide education to clients and families regarding health, medication, and safety

• Maintain accurate documentation in compliance with state and federal regulations

• Offer emotional support and compassionate guidance to clients and their families

Qualifications

• Active RN license in New York State (required)

• Minimum 1–2 years of nursing experience (home care or related field preferred)

• Strong clinical skills with the ability to work independently

• Excellent communication and interpersonal skills

• CPR and BLS certification (required)

• Reliable transportation and valid driver’s license

• Experience with Electronic Health Records (EHR) preferred
