Affirmed Home Care is seeking experienced Per Diem Pediatric Licensed Practical Nurses (LPNs) to join our exceptional clinical team in HARTFORD COUNTY, CT. About the Role

As an Affirmed Pediatric LPN, you’ll deliver one-on-one skilled nursing care to medically complex pediatric clients in the comfort of their homes. Our cases allow you to focus entirely on your patient—without the fast-paced demands of a facility setting.
We are especially interested in nurses experienced with medically fragile children, including those requiring g-tube, tracheostomy, and ventilator support to work in Hartford County.

Schedule: Overnight shifts, 8 hours (10:00 PM – 6:00 AM)

• Minimum 1 year of recent pediatric nursing experience (acute care, PICU, NICU, or home care strongly preferred)
• Required: Pediatric trach and ventilator experience
• Required: Pediatric g-tube experience
• Confidence caring for medically complex pediatric patients
• Active Connecticut LPN license
• Current BLS certification
• Eligibility to work in the U.S.
• Valid driver’s license
• Physical exam within 12 months
• PPD or QuantiFERON within 1 year, or chest X-ray within 5 years
