💼 Position Overview
As a Home Care Nurse at Affirmed, you'll provide direct care to patients in their homes — delivering the same high-quality nursing you'd expect in a clinical setting, but in a much more personal environment.
Responsibilities include:
Providing individualized nursing care based on each patient’s care plan
Administering medications, monitoring vitals, and managing complex needs as appropriate (e.g., wound care, catheters, colostomies)
Maintaining accurate and timely documentation
Offering emotional support to clients and families
Collaborating with the Director of Nursing to ensure high standards of care

📌 Qualifications
Valid RN or LPN license in the state of [Insert State]
Recent annual physical (within the last 12 months)
PPD or QuantiFERON test (or recent chest X-ray if applicable)
MMR titers/labs
