Licensed Practical Nurse (LPN) – Private Duty Case

Location: Chester, NJ

Affirmed Home Care is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide one-on-one care for a patient in Chester, NJ. This private-duty case requires strong clinical experience, professionalism, and a proactive approach.

Schedule Availability Needed:

• Monday–Wednesday: 4:00 PM – 7:00 AM

• Thursday, Friday, Sunday: 11:00 PM – 7:00 AM

• Saturday: 24-hour coverage needed

Clinical Requirements:

• Trach care

• Ventilator support

• Cough assist

• Suctioning

• Feeding tube management

• PureWick catheter care

• Hoyer lift transfers

Patient Overview:

• Bedbound

• Requires attentive, hands-on nursing care

• Medication management (reminders)

Equipment in Home:

• Hoyer Lift

• Hospital Bed

• PureWick Catheter

Additional Services:

• Respiratory Therapy currently in place

• Additional services being coordinated

Qualifications:

• Active NJ LPN license

• Experience with trach/vent cases strongly preferred

• Professional, reliable, and proactive

• Ability to provide safe, high-quality care in a home setting
