Make a Meaningful Impact in Pediatric Care
Affirmed Home Care is actively seeking an experienced New Jersey licensed RN, LSW, or LCSW with strong pediatric healthcare relationships to lead referral development and care coordination throughout New Jersey.
This is a high-impact opportunity for a clinically licensed professional with an established network across hospitals, pediatric practices, school systems, and specialty care providers.
You will serve as the primary liaison connecting medically complex pediatric patients and families to exceptional home-based care services while driving referral growth and strengthening provider partnerships statewide.

Required Qualifications
• Active New Jersey RN, LSW, LCSW license or related field in good standing (strongly preferred)
• Minimum 3+ years of pediatric care management, transition-of-care, case management, or clinical liaison experience
• Established referral relationships with NJ hospitals, pediatric practices, specialty providers, and/or school systems
• Strong working knowledge of:
• NJ Medicaid / NJ FamilyCare
• EPSDT
• IEP / 504 processes
• Pediatric specialty care networks
• Proven relationship-building and business development ability
• Excellent clinical assessment and communication skills
• Valid NJ driver’s license and reliable transportation
• Experience using EHR and care management software
Preferred Qualifications
• Bachelor’s or Master’s degree in Nursing, Social Work, Public Health, or related field
• CCM certification (or equivalent)
• Bilingual English/Spanish
• Experience working with medically complex pediatric populations, behavioral health, or special education services
• $150,000+ base salary, plus commission

Build and Expand Pediatric Referral Partnerships
• Leverage and grow an existing network across New Jersey hospitals, pediatric specialty groups, physician practices, and school systems
• Develop strategic partnerships with discharge planners, case managers, pediatricians, social workers, and school-based clinical teams
• Represent Affirmed at provider meetings, hospital rounds, community events, and educational in-services
• Track referral activity, conversion metrics, and growth opportunities
Coordinate Pediatric Care Transitions
• Assess pediatric patients’ medical, behavioral, psychosocial, and educational needs
• Develop individualized transition care plans
• Coordinate transitions from hospital to home, outpatient rehabilitation, school-based, or specialty care
• Monitor progress and adjust care plans as clinical needs evolve
Advocate for Children and Families
• Guide families through available services and benefits
• Support navigation of Medicaid, NJ FamilyCare, and private insurance
• Advocate for appropriate medical, therapeutic, behavioral health, and educational services
• Deliver compassionate, family-centered support
Ensure Clinical Excellence
• Maintain compliant, timely documentation
• Participate in interdisciplinary case reviews
• Support quality improvement initiatives
• Ensure adherence to NJ regulations and agency standards
• Pediatric RNs with strong discharge planning or case management backgrounds
• LSWs and LCSWs with pediatric transition-of-care experience
• Pediatric clinical liaisons
• Hospital discharge planners
• Clinical professionals with established NJ pediatric referral relationships
