Affirmed Home Care is looking for a compassionate and dependable Licensed Practical Nurse (LPN) to join our team and provide high-quality one-on-one care to a client in Metuchen, New Jersey.

What You’ll Do

Provide compassionate nursing care based on the client’s individualized care plan
Administer medications and prescribed treatments
Monitor the client’s condition and communicate changes appropriately
Provide wound, catheter, and other skilled nursing care within your scope of practice
Maintain accurate and timely documentation
Support the client’s comfort, safety, dignity, and independence
Communicate professionally with the client, family, and care team

What We’re Looking For

Active New Jersey LPN License
1+ year of nursing experience preferred
Current CPR/BLS certification
Strong communication and clinical skills
Reliable, compassionate, and professional
Comfortable working independently in a home care setting
Valid driver’s license and reliable transportation preferred
EHR/documentation experience preferred
