Hours: Monday – Friday | 8:30 AM – 5:00 PM
Patient Caseload: Approximately 20 visits per week (5 visits per day)

Training Program

All hired nurses will complete a comprehensive 3-week paid training program:

• 2 weeks of classroom training in Long Island City (LIC)
• 1 week of supervised field training

This program is designed to ensure confidence and success in the role before independently managing patient visits.

Key Responsibilities

• Provide skilled hospice nursing care to patients in their homes
• Conduct follow-up hospice visits
• Administer medications and treatments as ordered
• Provide wound care management
• Perform IV therapy and related interventions
• Conduct patient pronouncements when appropriate
• Monitor and manage pain and symptom control
• Collaborate with interdisciplinary hospice team members
• Educate patients and families on care plans and symptom management
• Maintain accurate and timely clinical documentation
• Provide emotional support to clients and families

Clinical Skills Required

Candidates should have hands-on experience with:

• IV therapy
• Midline catheter management
• Tracheostomy care
• Foley catheter care
• Ostomy care
• Nephrostomy tube care
• Tenckhoff catheter management
• Tube feeding administration
• Chest tube management
• Advanced wound care
• Pain and symptom management

Qualifications

• Active and unrestricted New York State Registered Nurse (RN) license
Bilingual in Spanish and English required
• Minimum 2 years of recent experience in one or more of the following:
 – Hospice
 – ICU
 – Med-Surg
 – Community Health
 – Oncology
• Strong clinical assessment, critical thinking, and care planning skills
• EMR proficiency (WellSky experience preferred; training provided)
• Recent physical exam (within 1 year)
• Current PPD/QuantiFERON (within 1 year) or chest X-ray (within 5 years)
• MMR immunization documentation
• Valid driver’s license and reliable transportation
• Authorized to work in the United States
• Ability to work independently in a field-based setting
