Licensed Practical Nurses (LPNs) – Flexible Per Diem Shifts
$40/hr & $125/Visit – Hartford County

Looking to make a real impact through one-on-one patient care? Affirmed Home Care is expanding our Hartford County team and seeking experienced Per Diem LPNs who are passionate about clinical excellence and personalized service.

We currently have openings in Hartford, East Hartford, Newington, Windsor, Manchester, and West Hartford. You choose the schedule that fits your life—whether it’s full-day support or shorter per-visit engagements.

Ideal Candidate Profile:

Active Connecticut LPN license
Minimum 1 year of clinical experience (hospital or home care)
CPR/BLS certification
Physical exam within the past year
Negative PPD or QuantiFERON test (or chest x-ray on file)
Reliable transportation and valid driver’s license preferred
Authorized to work in the U.S.
