Affirmed Home Care is urgently seeking experienced and compassionate Registered Nurses (RNs) to deliver exceptional in-home healthcare services across New York City and Long Island.

Compensation & Benefits

💲 Competitive Pay: $55 per hour, plus performance-based incentives
       RN Visits: | IV/Infusion $125 per case | Wound Care $125 per case

Key Responsibilities

• Perform comprehensive client assessments and develop individualized care plans
• Administer medications, treatments, and nursing procedures accurately
• Monitor patient health status and respond promptly to changes in condition
• Provide advanced clinical care including:
  • Wound care
  • IV therapy
  • Catheter management
• Educate clients and families on:
  • Disease management
  • Medication adherence
  • Home safety practices
• Maintain accurate documentation in compliance with regulatory standards
• Ensure adherence to all state and federal healthcare regulations
• Provide compassionate support and guidance to patients and families

Qualifications

• Active Registered Nurse (RN) License – New York State (Required)
• Minimum 1–2 years of nursing experience preferred
  (Home care, geriatrics, or related field strongly preferred)
• Strong clinical judgment and ability to work independently
• Excellent communication and interpersonal skills
• Reliable, organized, and self-motivated
• CPR/BLS Certification (Required)
• Valid driver’s license and reliable transportation
• Experience with Electronic Health Records (EHR) preferred
